On your first appointment at Rocky Creek Dental Care, you will need your insurance card and your ID, so that we can help you to utilize your benefits to the max.
If you do not have insurance, don’t worry we have specials to help you stay on track with your dental needs without breaking your budget.
After successfully scanning in your information into a patient file, we will take a photo of you to ensure your confidentiality and safety.
You will then fill out a Medical/Dental History form, a Patient Information form and a Notice of Privacy Practices (HIPAA).
All appointment changes must be made 24 hrs in advance. A $40.00 charge will be added to your account for appointments canceled without 24 hr notice.
At Rocky Creek we accept a variety of PPO insurance plans.
Please understand that we will provide an estimate to you, however it is not a guarantee that your insurance will pay exactly as we estimated. Any balance not covered by your insurance is your responsibility.
However, if at any point the insurance company or the insured becomes uncooperative, we reserve the right to terminate acceptance of benefits and collect payment directly from you.
Payments for services will be expected in full unless arrangements are made in advance of treatment.
Payments can be made with cash, ATM/Debit, Visa, MasterCard, Discover and American Express. A
ny check returned will be assessed a return check fee of $35.00. In the event that a delinquent account is turned over to an outside agency, the patient is responsible for the collection agency’s fees.
If financial arrangements are necessary after your first visit, we offer payment plans which can be customized to suit your needs. Any account with a balance remaining after 60 days will be assessed a fee of 18% annually, regardless of any outstanding insurance claims.
